Location: Based in Asthma Society Headquarters in Dublin, with an option to work from home one day a week
Hours: Part-Time (18.5 hours/ week)

Salary: Salary will be at Clinical Nurse Manager level 1, pro-rata
Contract duration: Initial contract is for six months, with a view to extension of contract as soon as funding is approved by Slaintecare following successful implementation of initial project plans.

 

The Programme Coordinator role will be to design, develop, launch and operationally manage the nurse led Asthma Society Slaintecare programme: Beating Breathlessness

The function of the Beating Breathlessness programme is to provide easily accessible support, education and information to people with asthma or COPD to help them better understand and manage their condition. The nurse led “Beating Breathlessness” project involves a two-pronged approach to asthma and COPD education and self-management:

The ASI WhatsApp Asthma & COPD Patient Programme.
The ASI Patient Asthma & COPD Prescription Self-Management Programme.

The Programme Coordinator will be a motivated self-starter, with a minimum of two years previous experience in the development of patient self-management programmes.

Overall objectives of Beating Breathlessness

•             To provide an easily accessible support, education and information to people with asthma or COPD to help them to better understand and manage their condition.

•             To provide advice and guidance on a different channel, reaching and supporting a younger cohort of patients and family members of COPD patients.

•             To empower people to engage better with healthcare professionals and healthcare services.

•             To sign-post (and provide) people with appropriate services, resources and materials.

•             To encourage people to look after their general health and well-being by encouraging smoking cessation, physical activity and promoting vaccinations.

•             To provide information and resources to members on an on-going basis through the ASI’s member network.

•             To reduce asthma deaths in Ireland.

Planned Beating Breathlessness Outcomes

•             People with asthma and COPD have a better understanding of their own asthma/COPD control and when they need to take action.

•             People with asthma and COPD understand their own individual triggers better and have better

•             People with asthma or COPD are empowered to better manage their condition through increased awareness of asthma or COPD management.

•             Improved understanding of medications and when to use them (i.e. difference between controller and reliever).

•             Improved knowledge of what to do in the case of an exacerbation - recognising the symptoms and the 5 Step Rule.

•             Improved confidence to engage with the health system, how to access and navigate it.

•             Decreased anxiety and improved confidence in effectively managing their condition.

•             Improved awareness of vaccinations in the management of asthma or COPD.

•             Increased development and rollout of asthma or COPD self-management plans, in collaboration with their GP or asthma nurse.

•             Increased take-up of the HSE’s Quit service.

 

Essential Requirements

Third level qualification in a healthcare field, such as health promotion, general nursing, community healthcare, with a strong knowledge and experience in project management.
A professional qualification and previous experience in people management.
A proven track record of creating and implementing patient focused programmes.
Previous experience of working within the HSE, private sector healthcare and/or Not for Profit is expected.
Experience in management of budgets and spend investment.
Computer Literate and an awareness of social media platforms and their functionality.

 

Desirable

A knowledge of implementing patient self-management theories and methods – such as the Stanford Self-Management Programme.

The Programme Coordinator will:

Generate, lead and coordinate the nurse led Beating Breathlessness programmes, including any nurse management, as appropriate. This will include overall responsibility for both Slaintecare Beating Breathlessness projects, as above.
Work closely with the Asthma Society’s Communications team and the Beating Breathlessness Working Group (patients and healthcare professionals) to ensure that all videos/graphics and key messaging is appropriate to meet patient needs.
Drive the growth and delivery of the service through patient and Health Care Provider engagement methodologies and engagement.
Communicate programme insights and recommend, growth improvements and/or solutions.
Ensure the asthma and COPD advice given to service users is evidence based and delivered in a professional ethical manner, in accordance with the NMBI code of conduct.
Ensure that the asthma and COPD advice given to service users is approved by the Medical Advisory Group of the Asthma Society, as clinical governance leaders.
Provide end to end project and programme management for all service users to include diversity and inclusion initiatives.
Contribute to the success of other team member initiatives.
Work closely with the Beating Breathlessness nurse to ensure the service delivery is efficient and meets patient needs.
Work with external information technology providers to ensure the services run on time and on budget.
Provide the ASI and POBAL with insights and reporting through analytics using the CRM reporting strategy and other relevant supporting determinants.
Manage and use the CRM Salesforce and WhatsApp, for reporting and data management.
Engage with Slaintecare funding recipients from other similar external organisations to share learnings and build mutually beneficial networks.
Present on behalf of and represent the Asthma Society from time to time on the Beating Breathlessness project, as required.
